Levels of Human Papillomavirus 16/18Neutralizing Antibody among 18~45 year-old Women in Liuzhou,Guangxi Province,China:Association with Precancerous Lesions of Cervical Cancer

阐明广西柳州地区自然人群中18~45岁女性人乳头瘤病毒(HPV)16/18型中和抗体和DNA流行情况,并探讨其与子宫颈癌癌前病变的相关性。201; 3年3月至7月在柳州市招募2; 300名18~45岁女性,采集血清以假病毒中和试验(PBNA)法检测HPV16/18型中和抗体,同时采集宫颈脱落细胞进行液基细胞学诊断和HPV; DNA检测,对细胞学异常者进行阴道镜检查,并对采集的组织学标本进行病理诊断。采用趋势性chi~2检验分析不同年龄段HPV; DNA阳性率及HPV中和抗体阳性率的差异,Logistic回归分析筛选宫颈癌前病变的影响因素。广西柳州地区18~45岁女性自然人群中,HPV16; DNA或中和抗体阳性364例(15.8%,95%CI:14.4,17.4),HPV18DNA或中和抗体阳性164例(7.1%,95%CI:6.1; ,8.3)。CIN3在不同年龄组的卡方趋势检验有统计学意义(P=0.005),; HPV16和HPV18型DNA阳性是CIN1+(宫颈上皮内瘤变1级及以上)、CIN2+(宫颈上皮内瘤变2级及以上)的主要危险因素,而自然感染产生; 的中和抗体与癌前病变未发现统计学相关性。HPV16/18型感染是宫颈癌癌前病变的主要危险因素,并未发现自然感染产生的中和抗体与其相关,表明接种疫; 苗仍是18~45岁女性预防HPV感染及癌前病变的主要方式。We wished to analyze the prevalence of human papillomavirus(HPV)16/18; neutralizing antibody, DNA and precancerous Lesions of Cervical; Cancer(PLCC)among women aged 18~45 years from the general population in; Liuzhou,Guangxi Province,China.From March to July,2013,totally 2,300; women aged 18~45 years were enrolled in Liuzhou.Neutralization of; HPV16/18 was detected by the pseudovirionbased neutralization assay and; Cervical exfoliated cells were collected for liquid-based cytological; and HPV DNA tests.Women were referred to colposcopy based on clinical; practice guidelines.The chi~2 test for trend was used to analyze the; prevalence of HPV DNA and the positive rate of HPV neutralizing antibody; in different age groups.The influencing factors of PLCC was analyzed by; logistic regression.Amongwomen aged 18~45 years from the general; population in Liuzhou,HPV16DNA and/or neutralizing antibodies were; positive in 364(15.8%;95%confidence interval(CI):14.4,17.4),andHPV18DNA; and/or neutralizing antibodies were positive in 164(7.1%;6.1,8.3).The; chi~2 test for cervical intraepithelial neoplasia(CIN)3 was; statistically significant in different age groups(P=0.005).Among samples; diagnosed as CIN1+and CIN2+,HPV16 and HPV18DNA positive was the main; risk factor.Neutralizing antibodies produced after natural HPV infection; make no statistically significant effect on Precancerous Lesions of; Cervical Cancer. HPV16/18 infection was a risk factor for; PLCC.Neutralizing antibodies produced after natural HPV infection had no; statistically significant effect onPLCC.HPV16/18DNA infection was a risk; factor for PLCC. These data suggest that a HPV16/18 vaccine would be the; primary choice to prevent HPV infection and the precancerous lesions in; women aged 18~45 years.
